人们俗称的乙肝“大三阳”，实质上是指乙肝病毒的HBeAg阳性，而“小三阳”则是指抗－HBe阳性。“大、小三阳”的共同点在于二者都是现症病毒感染者，体内都携带有乙肝病毒，区别则在于“大三阳”者病毒复制活跃，病毒数量多，传染性强；“小三阳”则是病毒复制低下，病毒数量少，传染性低（当然，不包括病毒基因前C区变异的患者）。所以说，“大、小三阳”只是在携带乙肝病毒的数量上有差别，而不是有病毒和无病毒的差别。简单说就是，无论“大、小三阳”都携带有乙肝病毒，都有传染性，但传染性的大小不同。
乙肝携带者可以是单独1阳性、15、145、1345阳性其中一种，其中145阳性的携带者与小三阳主要区别是携带者的hbv-DNA阴性，一般不具有传染性，但携带者发病后会有传染性，所以日常接触也需要注意的
简单来讲，就是乙肝两对半中，第一项阳性，其余阴性即为乙肝携带者；一四五阳性，其余阴性即为小三阳；一三五阳性，其余为阴性即为大三阳。表面携带者没有传染性，小三阳传染性不强，大三阳传染性强，病毒复制活跃。
大三阳的传染性比较强，病毒处于快速复制期，小三阳传染性比较弱
