心脏的杂音都是瓣膜功能异常、瓣膜结构异常、房室间隔异常、乳头肌功能异常或者血液流速异常而造成的，三级杂音只是一个分级。
心脏如果出现三级以上的杂音，可能就是病理性的杂音了，尤其是舒张期的杂音，大多数都是病理性的。出现这种情况，建议进一步查心电图和心脏彩超，看是否有先天性心脏疾病或者是心脏瓣膜的疾病。如果没有问题的话，观察病情变化，有问题的话，根据具体的结果进行进一步的检查。
医学资料表明健康小儿有23%-60%在胸骨左缘或心尖部可听到这种杂音，从新生儿，婴幼儿直至学龄儿心脏可有Ⅰ-Ⅱ级收缩期轻微的杂音，其性质较柔和如吹风样“夫一夫”音，有的呈乐音样.这种杂音的产生与小儿代谢旺盛，心肌收缩有力及血液循环较快有关，一般到12-13岁时就会消失，孩子无任何不适，活动也不受影响.经医生及X线，心电图，超声心动图等检查都缺乏心脏病的证据.这种生理性杂音多在偶然情况下，如进行体格检查时或有其他疾病时发现，因此有生理性杂音并不是得了心脏病，不需要治疗。
病理性杂音是由于心脏备管发生病理改变(包括先天性和后天获得性)而出现的杂音，这种心脏杂音一般较粗糙，响度达Ⅲ级或Ⅲ级以上，病儿活动时可出现气促，无力等不适，常见的心脏病有先天性心脏病，风湿性心脏病及感染性心内膜炎等心脏病.但是有时生理性杂音偶尔可达Ⅲ级，不易和先天性心脏病和风湿性心脏病引起的杂音区别，必须进一步作有关检查.近年来随着超声心动图的发展，尤其是彩色多普勒超声心动图的应用，一般可以鉴别出小儿心脏杂音的性质是属于生理性还是病理性的，并能了解生理性杂音发生的原因。
